We have joined with Bawtry & Blyth Medical to form a Larwood & Bawtry PCN. This covers patients registered with Larwood Health Partnership, Westwood Surgery and Bawtry & Blyth Medical.
PCN contracts were offered to practices with between 30,000 – 50,000 patients, or for groups of practices covering this number of patients.
The aim of the PCN is to create a workforce within primary care not purely based on the traditional view as GPs and nurses. Each PCN create a team which best suites the need of their population. Our team now includes Clinical Pharmacists, Paramedics, Pharmacy technicians, Social Prescribing Link Workers, Care Co-ordinators, First Contact Physiotherapist, Trainee Nurse Associate and will grow as new roles are made available.
We work together under the leadership of our Clinical Director to provide the best and most appropriate care that we can for our collective patient list. This includes working across different organisations including, voluntary sector, local hospital, district and county council to name a few.
